     You Must Be At Least 18 Years Old To be Able To Start Any Semester

   Your Parent Or Guardian Can Make A Personal Payment Plan If Your Under 18

   You May Take Over The Personal Payment Plan Payments When You Turn 18

     (These tests are needed before you can start your 1st semester)

Required Tests :  Hepatitus C,   H.I.V.,  T.B.,  S.T.D.  (for sores)

Required Shots :  Hepatitus B,   Meningitis

                            Hepatitus B

1.   1st Shot - immediately before you start your 1st semester.

2.   2nd Shot - 1 month (or as required) after your first vaccination is recieved.

3.   3rd Shot - 6 months (or as required) for your 3rd vaccination.

                            Meningitis  

MUST BE RECIEVED EXACTLY 10 DAYS BEFORE YOU START YOUR 1ST SEMESTER 

                   (10 day incubation period)

All of the required tests & all of the required shots must be done through our facility unless we approve another alternate agreement. We will need all of the original test results that no symptoms exist & we will always need the original shot records of all of the vaccinations that have been recieved. All of the required tests & all of the required shots must be kept current. All of the required tests & all of the required shots must be done at your expense.

               If You Are Late For A Personal Payment Plan Payment

                      There Is A  $ 5  Per Day Late Fee Charge

   If You Are More Than 30 Days Late For A Personal Payment Plan Payment

      You Are Disqualified From Your Course & From Your Personal Payment Plan

If The 60 Days Expires, You Don't Owe Us Any Late Fees. You Must Start Over On A Brand New Personal Payment Plan. Your New Personal Payment Plan Will Be Towards A Brand New Course. You Must Register, Rebuild Your Schedule, Bring All Documentation That Is Required For Registration, All Of Your Required Tests & All Of Your Required Shots Must Always Be Kept Current...

You Will Get An Additional 60 Days To Pay All Of The Late Fees That Are Owed & To Catch Up To Where You Need To Be On Your Personal Payment Plan Payments
